All parents must print and fill out the forms to bring at the Parent/Player Meeting. All forms are necessary and mandatory and must be filled
out priorto coming so we can start our meeting on time. If you are not attending our meeting, the forms must be at our home office prior to
our parent meeting date.
Meeting will take place: Community Christian Church in Plainfield - 24035 W Riverwalk Ct, Plainfield, IL 60544 Monday - June 17 (All Teams EXCEPT High School Boys Teams. Meeting will be in October for High School Boys)
Uniform Fitting 5:45pm - 6:30pm Completed Paperwork Registration/Payment Line 5:45pm - 6:30pm Meeting starts promptly at 6:30pm - 8pm.
If you child has never played in a traveling club team before, please bring a copy of your child's birth certificate. This is required to
register your child. We will also need a photo for their player passes but we can take one at the parent/player meeting for you. Please ring
your child/player with you to this meeting.
